Telehealth Consent

This Telehealth Consent (“Consent”) governs your use of telehealth services provided by Key Health LLC (“Key Health,” “we,” “us,” or “our”). Please read it carefully.

Key Health LLC is a telehealth practice that provides clinical services through state-licensed healthcare providers. By accessing or using Key Health’s telehealth services (the “Services”), you acknowledge that you have read, understand, and agree to the terms of this Consent.


1. What is Telehealth?

Telehealth allows licensed healthcare providers to assess, diagnose, and treat patients remotely using technology, which may include:

  • Secure electronic intake forms
  • Two-way live video and/or audio communications
  • Remote transmission of medical records, photos, or health-related data

Key Health uses HIPAA-compliant systems with encryption and security protocols designed to protect your health information.


2. Benefits and Risks of Telehealth

Potential Benefits

  • Convenience: Care may be delivered without requiring travel to a physical office
  • Accessibility: Flexible appointment times and access to licensed providers
  • Privacy: Secure, remote consultations from your preferred location

Potential Risks

  • Limitations: Lack of an in-person physical exam and/or limited access to your full medical history may reduce diagnostic accuracy
  • Technical Issues: Service interruptions, video/audio failure, or loss of data may impact care
  • Security: Although safeguards are used, privacy breaches or unauthorized disclosures may occur
  • Legal/Regulatory Restrictions: Some states restrict telehealth or limit prescribing certain medications remotely

3. Your Responsibilities and Understanding

By using the Services, you agree and acknowledge:

  • You are receiving telehealth services from a licensed healthcare provider (which may include a Nurse Practitioner)
  • You understand you may be treated by a nurse practitioner rather than a physician
  • Providers determine whether telehealth is appropriate on a case-by-case basis
  • You are responsible for providing a complete and accurate medical history, including all prescriptions, over-the-counter medications, supplements, allergies, and diagnoses
  • Failure to disclose relevant information may result in improper care, adverse outcomes, or harm
  • Your provider may recommend in-person care and/or refer you to an urgent care, emergency department, or your primary care provider when appropriate

Eligibility & Location: You must be 18 years or older and physically located in a state where Key Health providers are licensed at the time of your visit. Key Health currently serves patients located in:
NY, DE, VT, FL, IA, ND, SD, NE, KS, OK, TX, CO, AZ.


4. Consent and Withdrawal

  • Your use of the Services constitutes your voluntary consent to receive care via telehealth
  • You may withdraw consent at any time by contacting your provider in writing
  • Withdrawing consent will not affect care already delivered or required documentation
  • Continued use of the Services affirms your ongoing consent

First Visit Requirement: Key Health requires the first visit to be a live, face-to-face virtual visit to establish the patient-provider relationship.


5. Emergency Situations

Key Health does not provide emergency care.

If you are experiencing a medical emergency, severe symptoms, allergic reaction, or an adverse drug event, call 911 or go to your nearest emergency room. Telehealth is not appropriate for life-threatening or urgent situations.


6. Privacy and Security

  • Information transmitted electronically will be managed in accordance with applicable privacy and security requirements, including HIPAA
  • Key Health providers are required to maintain confidentiality and safeguard your protected health information

7. Financial Terms

  • You are responsible for all costs associated with the Services, including consultations, prescriptions (if applicable), and medication fulfillment
  • Key Health is a cash-pay practice and does not bill insurance, Medicare, or Medicaid
  • Appointment fees are paid before your visit.
  • Visits are $50 per appointment. Medication costs vary based on your prescription and will be discussed by your provider.
  • You may choose to use an HSA card if eligible.
  • Depending on the medication prescribed, it may be sent to your local pharmacy for pickup or shipped to your home from a dispensing/compounding pharmacy partner when appropriate.

Acknowledgment

By proceeding with the Services, you confirm that you understand and agree to the terms of this Telehealth Consent and consent to telehealth treatment from a provider licensed in the state where you are located at the time of your visit.
